To create templates of forms with the testing information prefilled in, go to:
Setup, Charting Setup, Setup Form Templates:
Select New to find the form that you would like to make a template of from the Select Style window:
*Note: you may need to select the Show All Forms checkbox to find the original form
Rename the form title to identify the form according to how you have it customized (ie. Dynalife Routine Female Over 50):
If this form is customized according to a specific physician’s ordering preferences, then select the physicians name from the dropdown list so it will only be visible to them.
Select the applicable tests on the form and click Save.
To use form on a patient, select from Form Templates in top right portion of selection window instead of from the standard forms on left side:
