Forms are one of Healthquest’s major features, with over 3000 available. However, you may run in to a situation where the form you require is not in Healthquest, or you need to update an existing form.

To request an addition of, or update to, a form, fill out the Microquest Form Request. Within Healthquest, create a new form and search for “Microquest Form Request.”

Along with the request, a copy of the new form, or the existing form with the changes clearly indicated, must be included.

You can send a PDF version of the request and the form to forms@microquest.ca, or fax paper versions to 780-434-6018.

Most form requests require 4-6 weeks to complete. Urgent requests will be assessed on a per-request basis.
